Enrolling Students in the Sunnyvale School District

During the Current School Year:

If you know your child's home school, (the school closest to your residence), please take the following information to the home school to enroll your child:

  1. Proof of Residence: You must have one of each: (originals only; no copies accepted)
  • Current PG&E Bill (for mobile homes: space rental bill)
  • A Picture Identification such as a driver's license or passport.
  • 1 of the following: Pay Stub or Pay Warrant; Water / Sewer / Trash Bill; Lease Agreement / Mortgage; Federal / County / State Mail; Car Registration; Car / Home Owners Insurance Bill; Bank Statement w/ voided check; Medical or Credit Card Bills
  1. Proof of Current Immunizations

  • 3 Polio (last given after the age of 4*)

  • 4 DPT (last given after the age of 4*)

  • 2 MMR (after the age of 1* )

  • 3 Hepatitis B (Prior to Kindergarten entry, and 7th Grade Entry)

  • 1 Chickenpox (Prior to Kindergarten, or for any child coming from another State or Country)

  • * or 4 days prior to age 4

  1. Proof of TB Test--Given within 18 Months Prior to Kindergarten Entry, or For any Child Coming from Outside Santa Clara County--Kindergarten through Eighth Grade.
  • Mantoux (PPD) test results (date given, date read, and by whom)
  1. Verification of Date of Birth: You may use:
  • Birth Certificate
  • Baptismal Certified
  • Passport
  • Statement by local registrar or county recorder certifying date of birth
  • Notarized affidavit of parent/guardian
