2004 – 2005 Sunnyvale Spartan Athletics
Dear Athletes and Parents:
Congratulations! We welcome you to our after-school sports program. As coaches, we feel it is necessary to explain in some detail what is expected of our student-athletes.
GPA: California State law mandates that student-athletes maintain a 2.0 Grade Point Average. If the students fail to meet the 2.0 GPA from the previous report card period, they will be unable to participate in sports. The students receiving a “U” mark in their citizenship grade will also be unable to participate.
Practice: All of our student-athletes are expected to be present at all practices held at Sunnyvale Middle School. Please refer to the following information regarding practice:
1. If a student-athlete cannot attend practice/game for any reason(s), it is his/her responsibility to notify the coach.
2. One unexcused absence results in not playing the following game. Two unexcused absences generally lead to dismissal from the team.
3. Please make dental, medical, etc. appointments after practice or on another day.
4. All student-athletes will help with set up and clean up of the equipment.
Fee: A check made out to the CITY OF SUNNYVALE in the amount of $55 must be submitted to your coach along with all the paperwork. Your son/daughter cannot play until all the paperwork and a check is submitted. If this fee is difficult for you to manage, please contact the coach of the team or our Athletic Directors: Mr. Tsurushima or Mr. Hernandez. Please understand that a refund will NOT be given if:
1. An athlete is dropped from the team due to misbehavior, missed practice or discipline.
2. An athlete decides to quit the team.
3. After gaining playing privileges through probationary grade check, subsequent grade checks render the athlete ineligible.
Uniform: All of our student-athletes will be issued a uniform and it is his/her responsibility to return them to his/her coach by ____________________. If the uniform is lost or damaged due to negligence, the athlete or the parent must pay for the uniform. So, take care of them!
Good Sportsmanship: Student-athletes must realize that they represent their schools, coaches, family, and themselves by their conduct. Therefore, good sportsmanship is mandatory. An athlete who practices poor sportsmanship may be eliminated from the team.
Transportation: We rely on parent-volunteers to transport our student-athletes to our away games/meets. We greatly appreciate all the parents who can volunteer for this important duty. If you are available to drive, please fill out the “School Driver Certification Form.”
